For radio buttons, set Name = custom_[fieldName] on each radio button in the group. Set the proper value of the custom field on each radio. The checked radio will pass value as the custom field. On the CRM side, the custom fields must be set to “Text”. These custom text inputs will display the value assigned to the radio buttons in the in the CRM.
