Go to Admin | Plugins | Shipping Services | Available Plugins: "Insureship" and click the Green Activate Button:

 

Enter your Insure Ship API information into the dialog box:

Go to CRM | Campaigns and select the campaign you want to add Insureship to. Under General Details, select Offer Insureship:
